All of the information I have given is true and complete. I will provide it with quality dog food, plenty of fresh water, shelter, affection, annual physical examination and vaccinations.

**The completion of this application does not reserve your puppy. Once your application is approved and you have paid the non-refundable deposit fee of $300, the puppy of your choosing will be reserved for you. The remaining full balance of $1450 ($1300 + 8.45% tax) is due on the day you pick up your puppy.
